Primary Care at Home
Checkups, sick visits, lab orders and treatment plans without the trip across town or the hours in a waiting room.
About primary care →Yes, Seniority Healthcare provides in-home medical care for older adults throughout Montgomery County, from the Main Line and Norristown to Abington, Horsham and Pottstown. Every patient gets a physician-led care team (a provider, a nurse case manager and a care coordinator) that manages primary care, chronic conditions, medications and behavioral health at home, so your parent can stay in the house they love without a calendar full of office visits.
Search “home care Montgomery County” and you'll find dozens of agencies offering aides for bathing, meals and companionship. Seniority is something different: medical care at home, led by a physician, that keeps your parent's health on track.

Already have a home care aide or home health? That's fine. Our care works alongside them and won't interfere with those visits or your loved one's other appointments.
Montgomery County families are often managing a parent's care between work, kids and a commute on the Schuylkill or the Turnpike. Seniority gives your loved one one team that handles the coordination and keeps you updated.
Whether your parent lives on the Main Line or in North Penn, these services come to the house and work together under one care plan.

About primary care →A dedicated nurse who knows your parent, visits regularly, tracks vitals and symptoms, and spots problems before they become emergencies.
About nurse case management →Support for depression, anxiety and dementia-related behavior changes, built into the same care plan instead of a separate referral.
About behavioral health →Nearly one in five Montgomery County residents is 65 or older, spread across dozens of townships and boroughs.
Montgomery County's older adults live in every kind of community: Main Line neighborhoods in Lower Merion and Narberth, established suburbs like Abington, Cheltenham and Upper Moreland, river towns like Norristown, Conshohocken and Pottstown, and quieter townships farther north and west. Many have lived in the same house for decades and want to stay there. The hard part is everything that comes with it: specialist appointments at different hospitals, a growing list of medications and a parent who is no longer comfortable driving.
Seniority brings the medical side of that to the house. A nurse case manager visits on a regular schedule, the provider directs the care plan, and a care coordinator keeps specialists, pharmacies and family on the same page. When something changes, there's a provider available 24/7, not a voicemail.

After that, the nurse case manager makes the regular visits to your parent's Montgomery County home, and the provider directs the plan, using telemedicine when it helps.
Care usually starts within one to two weeks of your first call. We confirm coverage first, schedule the first home visit, then visit weekly, every other week or monthly, based on your parent's needs.
