"I have been working with Seniority for more than five years. I first approached Dr. Hopkins when one of the communities I support needed medical support. This particular community is in a remote location and in need of a responsive physician. Seniority presented with an innovative chronic disease management program and included the use of telehealth to support a virtual care platform with 24/7 access to a clinician.  In the first three months of our partnering with Seniority, the community saw improved outcomes among our resident population including:  a significant reduction in unnecessary emergency transfers; improved glycemic management of residents with diabetes; improvement in management of residents with cardiac disease and congestive heart failure.  During the ensuing years, Seniority has become an established presence in additional communities. A practitioner comes to the community one or more days every week to conduct wellness and “sick” visits. The practitioners have access to a resident’s electronic medication records, which promotes another best practice to ensure ongoing medication regimen reviews. This is particularly important when more than one prescriber is involved in the residents’ medication management. In addition, Seniority has been provided access to the online ordering system for diagnostic testing, which promotes definite efficiencies on the ordering end as well as on the interpretation of results.  The expected improvements in resident care have become the norm with Seniority in the community. "I have been fortunate to partner with Seniority Healthcare over the past 14 months in Covid-19 outbreak response operations across the South Central region of PA. Seniority has been innovative, forward thinking and committed to delivering the most impactful care to residents in long term care communities. When monoclonal antibody therapy became available, before vaccines had begun to be administered to older adults living in congregate care settings in December 2020, Seniority immediately stepped up with a skilled team to deliver monoclonal antibody infusions in the nursing homes and personal care homes in partnership with the team I lead at Penn State Health. This was life saving treatment for older adults then, and they continue to support this important work for our most vulnerable older adults.
